Position Overview:

The clinical care coordinator will work with a multidisciplinary team to provide individualized person-centered services to adults with a severe mental illness. This is a community-based program with 50% of the services will take place in the community or in the clients’ home. The work includes individual counseling, case management, groups counseling, family support group, skills training, and coordination with community providers to assist the client in achieving identified goals, improving overall wellness and reducing hospital and jails stays.

Essential Job Functions:

Main Duties:

  • Delivers therapeutic services using evidence based, best practice CBT, DBT, Motivational Interview, Trauma Focused-CBT, approach with individuals, families and groups as determined by the individual through their treatment plan.
  • Provide ongoing assessment and treatment planning, case management, supportive psychotherapy, symptom management, medication monitoring for adults with a severe mental illness.
  • Use the Recovery Model philosophy to guide the interactions with enrolled clients.
  • Support clients’ recovery through activities of daily living to include coaching and hands on assistance in the areas of housing, money management, vocational/educational pursuits, socialization, and scheduling/transportation to appointments whiles addressing mental health symptoms that interfere with these daily life tasks.
  • Coordinate services and communication with community providers such as PCP, probation and parole, housing authority, Washington County crisis team, MHRT, etc..
  • Provide crisis prevention and intervention services by following crisis protocols. Responsible for answering a 24/7 support phone on rotation among team members.
  • Responsible for psychoeducation and coordination of care with family members and natural supports individually and through a family support group.
  • Completes accurate clinical documentation within the timelines and guidelines consistent with agency standards, rules and regulations; including diagnosis using the DSM V. Use collaborative documentation whenever possible.
  • Meet with Supervisor minimum of 2 hours per month. Inform Supervisor immediately of any serious issues. Complete Incident Reports in a timely manner (within 24 hours). Adhere to all mandatory abuse reporting laws.
  • Communicate individuals’ changes in behavior, symptoms and/or functioning to Supervisor and medical professionals.
  • Facilitate appointments with psychiatric and medical providers. Communicate regularly with psychiatric and medical providers regarding any reported side effects or issues about medication shared in counseling session for improved care coordination. At times, helping a client learn the names and sight of medications through setting up weekly cassettes or reviewing bubble packed medications.
  • Demonstrate understanding of ethical standards, clients’ rights and responsibilities and professional boundaries.
  • Coordinate with any additional inside or outside treatment providers such as Substance Abuse Providers, Supported Employment, Vocational Rehab, etc.
  • Attend and participate in group supervision, monthly all staff meetings, in-services and other departmental meetings
  • Complete annual training requirements by attending seminars and other educational opportunities to develop professional skills and abilities.
  • Other job duties as assigned

Required Skills:

  • Experience with a Trauma Informed Recovery focused philosophy.
  • Familiarity with DSM V and ability to diagnosis mental health disorders.
  • Ability to respond appropriately to crisis situation.
  • Good organizational and computer skills; previous experience with electronic medical records preferred.
  • Effective time management skills and ability multitask
  • Knowledge and/or experience with community based rehabilitative case management services within a mental health management care system.
  • Ability to work with individuals who have experienced trauma in their lives and have not yet learned how to communicate in healthy ways or do not have the capacity to communicate in health ways due to a severe mental illness or intellectual disability.
  • Commitment to work with individuals who are struggling with safe interpersonal interactions that can at times look like anger, yelling and volatile gestures.
  • Knowledge and comfort with psychopharmacology terminology.
  • Maintain current Oregon or Washington Driver’s License and vehicle insurance as required by state law.
  • Support the organization's mission along with a workplace culture of diversity equity and inclusion.
  • Management of work processes along with strong computer skills for documentation purposes.
  • Ability to maintain confidential information within HIPAA guidelines and organizational policies.
  • Ability to be flexible especially with scheduling and site locations and to perform and prioritize multiple functions and tasks.
  • Ability to speak Spanish is a plus, pay differential offered.
  • Excellent communication skills.
  • Ability to pass a criminal history check

Physical Requirements:

Education:

  • Masters’ Degree from accredited university in human/social services field (social work, counseling, psychology) required.
  • Must meet qualifications for a Qualified Mental Health Professional (QMHP).
