FEP Social Worker, Senior
Job Description
Your Role
The Federal Employee Program (FEP) team performs integrated case management (CM) and disease management (DM) activities demonstrating clinical judgment and independent analysis, collaborating with members and those involved with members' care including clinical nurses and treating physicians. The Social Worker will report to the FEP Department Manager. In this role you will be part of a multidisciplinary team that determines, develops, and implements a plan of care based on accurate and comprehensive assessment of the member's needs.
Your Work
In this role, you will:
- Collaborate with case managers and other team members to complete member assessments related to social and resource needs to support member access to services
- Collaborate with providers to support members treatment plan and care needs
- Recognize the client's right to self-determination as it relates to the ethical principle of autonomy, including the client/family's right to make informed choices that may not promote the best outcomes, as determined by the healthcare team
- Assess members health behaviors, cultural influences and clients belief/value system and evaluates all information related to current/proposed treatment plan and in accordance with clinical practice guidelines to identify potential barriers
- Effectively manage and maintain a caseload of members with high intensity of needs such as members experiencing homelessness, poor social support, mental illness or polysubstance abuse
- Engage with case managers, discharge planners, members, and/or members' support system in planning safe discharges and transitions between settings
- Provide telephonic support to members, and/or members' support system, by connecting them to appropriate supportive services and/or benefits
- Actively participate in interdisciplinary care conferences and case consultations by providing clinical knowledge and support for social work activities
Your Knowledge and Experience
- Master's Degree in Social Work (MSW) or related field from an accredited university required
- Licensed Clinical Social Worker (LCSW) or equivalent licensure (e.g. LPCC, LMFT, etc.) preferred
- Case Manager Certification (CCM) preferred
- 3 or more years of experience working with the Medi-Cal population, medical social work, community clinics, or other home and community-based organizations
- Strong critical thinking and analytical skills reflecting independent problem identification and resolution
- Experience conducting thorough psychosocial assessments, developing care plans and providing necessary interventions identified during assessment with complex client populations
- Thorough knowledge of services provided by, and eligibility for, applicable state and federal programs in addition to local, community-based organizations required
- Considerable knowledge of a wide range of psychosocial challenges and familiarity with behavioral health diagnoses and treatment preferred
- Proficient with computer programs such as Microsoft Excel, Outlook, and Word required
- Must be able to sit for extended periods of time and read information on one computer screen and apply that information on a second computer screen to complete documentation
Pay Range:
The pay range for this role is: $ 77990.00 to $ 116930.00 for California.

Please note that this range represents the pay range for this and many other positions at Blue Shield that fall into this pay grade. Blue Shield salaries are based on a variety of factors, including the candidate's experience, location (California, Bay area, or outside California), and current employee salaries for similar roles.
